Immunization Guidelines
Dpt, DTaP  5 doses unless 4th dose was given on or after 4th birthday.  Last dose, whether 4th or 5th, must be given on or after 4th birthday.

7 years and older: 3 doses required with the 3rd dose on or after 4th birthday.
Polio 4 doses unless 3rd dose was on or after 4th birthday.  Last dose, whether 3rd or 4th, must be given on or after 4th birthday.

7 years or older 3 doses required with 3rd dose on or after 4th birthday.
MMR 2 doses with the 1st one on or after the 1st birthday.  2nd dose completed before entry into kindergarten.
Hepatitis A 2 doses on or after 1st birthday grades K-3.  Applies to Grayson County. THIS APPLIES ONLY TO K-3.
Hepatitis B 3 doses required grades K-12.
Varicella(Chicken Pox) 1 dose on or after 1st birthday grades K-12.  2 doses are required if the child is 13 years old or older at the time the first dose was recieved.
